Is this really the correct representation for Dolby Digital 4.0?
I thought 4.0 meant 2 front channels and 2 rear (surround) channels, no center channel.

In Dolby Digital 4.0 there is encoding for four discrete sound tracks through the AC-3 bitstream. When played through Dolby Digital equipment, sound is heard from all five speakers. The surround sound information is discrete monophonic. This means there is a separate channel devoted to the rear speakers but the information is sent to both rear speakers. The sound is very similar to Prologic giving less definition to the rear of the sound field. There are only a small number of titles in this format. There is also no sub woofer track present.

Ok, I buy that.

However, I know I have read about a Dolby variant that uses the 4 channels in the manner I described. What would be the name for that?

Quadraphonic sound

(When transferred to Dolby Digital it would probably use the 5.0 or 5.1 configuration.)

Yes, it's quadrophonic, but is there a Dolby name for that encoding?
Or would it always be encoded as 5.something?

Well, if you have the Immersion release of Pink Floyd's Dark Side of the Moon you can check what encoding they have used, I believe it contains the original quadraphonic mix.
