The way I read this

Alternate version from theatrical and (no rating or "NR" or "Not Rated")

Or

Unrated anywhere.

It cannot just simply be a different cut but must also include one of the 2 conditions (no rating or "NR") in the first line, or be unrated per second line.

I think it is clearly rated R for the movie. But should it carry an unrated designation because the motion comic is unrated?

The Motion Comic is not rated, not unrated. Also, it's a special feature, so doesn't count.

Exactly. The main feature is what the rating is for and in this case it is rated R.

We profile DVDs not movies. 

(I'm just saying that because there was a case of the german version of Lord of the Rings FOTR that got an FSK-16 rating because of one bonus feature. When sold as single disc version it got an FSK-12.)

While the rating in Germany, UK and probably in most other countries around the world is given for the complete package on disc, the US rating shown on the package is given for a single feature only. That's also the reason why a US package can have more than one rating on the cover and we have to use the highest one for profiler.
