Regarding the columns, I think it has to be taken case by case since sometimes they're clearly meant to be read one way or the other.  If you can make a judgment as to which way the flow is intended (if they're alphabetical if read one way, or are in order of importance in the case) I'd follow the flow.  Otherwise I guess I'd go across rather than down first.
